US student
1. Application completed and application fee submitted.
2. Personal statement required with application
3. Official transcripts from all-post secondary institutions attended
4. Three references / recommendation letters have been requested.
5. Current resume sent or uploaded (if applicable)
International Student
In additon to the above requirement, the followings are needed:
1. Name field must match with your passport exactly.
2. Form is signed by the student.
3. Photocopy of your passport
4. TOEFL test score directly from the institution.
5. Financial statement signed by sponsor
6. Dependent (spouse or children) documents if you accompany them
